BackgroundRadiotherapy is an essential component of cancer treatment, yet many countries do not have adequate capacity to serve all patients who would benefit from it. Allocation systems are needed to guide patient prioritization for radiotherapy in resource-limited contexts. These systems should be informed by allocation principles deemed relevant to stakeholders. This study explores the ethical dilemmas and views of decision-makers engaged in real-world prioritization of scarce radiotherapy resources at a cancer center in Rwanda in order to identify relevant principles.MethodsSemi-structured interviews (...) were conducted with a purposive sample of 22 oncology clinicians, program leaders, and clinical advisors. Interviews explored the factors considered by decision-makers when prioritizing patients for radiotherapy. The framework method of thematic analysis was used to characterize these factors. Bioethical analysis was then applied to determine their underlying normative principles.ResultsParticipants considered both clinical and non-clinical factors relevant to patient prioritization for radiotherapy. They widely agreed that disease curability should be the primary overarching driver of prioritization, with the goal of saving the most lives. However, they described tension between curability and competing factors including age, palliative benefit, and waiting time. They were divided about the role that non-clinical factors such as social value should play, and agreed that poverty should not be a barrier.ConclusionsMultiple competing principles create tension with the agreed upon overarching goal of maximizing lives saved, including another utilitarian approach of maximizing life-years saved as well as non-utilitarian principles, such as egalitarianism, prioritarianism, and deontology. Clinical guidelines for patient prioritization for radiotherapy can combine multiple principles into a single allocation system to a significant extent. However, conflicting views about the role that social factors should play, and the dynamic nature of resource availability, highlight the need for ongoing work to evaluate and refine priority setting systems based on stakeholder views. The author argues the following hypothesis: the medical clinic is an experimental practice, in the sense given to this term by Claude Bernard, and the clinic is its specific laboratory. Its object is not the disease, but the patient. Careful examination of the clinic attests to its very close proximity to the experimental method, and the comparison also raises a number of difficulties. The main obstacle arises from the specificity of medicine, which involves treating individual human subjects whose words cannot (...) be suppressed. Its main challenge is to combine a linguistic practice with an experimental method. (shrink)

The purpose of the article is to show how the questions of anomality and animality belong together in phenomenology. The figure of the human animal serves as the guideline of the study, namely the figure of a person who is not considered as similar to myself in the frame of a Husserlian characterization of normality. Husserl’s thinking is analyzed with respect to the problem of an intersubjective co-constitution of a common world. It is shown that Husserl only accepts animality and (...) anomality within the restrictive perspective of a so-called normal humanity, that of the adult man, healthy, reasonable and European. However, he constantly strives to include the anomality and animality into humanity as well, an intention that distinguishes him from Heidegger. Heidegger radicalized the question with respect to race, which serves him as a guideline for thinking the very own of the human being. Defending a thinking of exclusion, Heidegger refuses to animals as well as to anomals all right of citizenship in the community of human beings. The article ends by proposing to overcome this duality between an inclusive thinking and an exclusive thinking. (shrink)

Renonçant à réduire l'art à la production d'une représentation dominée par le plaisir de la vue, le mouvement esthétique de la géopoétique cherche à faire de la création artistique un geste de participation aux matières de la terre sur fond d'une expérience polysensorielle où la marche et le nomadisme constituent des explorations d'un proto-monde. L'oeuvre écrite ou plastique est (...) destinée à conserver des fragments et traces de cette aventure sensible, ouvrant ainsi la voie à une rencontre qui n'est plus un spectacle mais une expérience quasi chamanique. Reste à savoir dans quelle mesure on peut s'émanciper totalement de la représentation pour accéder à une auto-présentation du cosmos dont l'artiste serait le médium. (shrink)

The literature on organizational ethics has paved the way for a situated and intersubjective understanding of ethics through caring practices. In this article, we try to extend this perspective by looking beyond the interactions of caregivers among themselves or with care seekers to reveal ethics as the ongoing collective accomplishment of a variety of actors. We do so by mobilizing Strauss’s theoretical perspective of articulation work in the context of healthcare. Based on an ethnography, we show how actors of care (...) (e.g., nurses, caregivers, or doctors) operate within care arrangements that need to be frequently rearticulated to face ethical threats and protect their patients’ well-being. We distinguish three types of rearticulation corresponding to different degrees of rearticulation, individual actor stance, and specific ways of working things out. We discuss their implications for the maintenance of organizational ethics. (shrink)

Most previous research on human cognition has focused on problem-solving, and has confined its investigations to the laboratory. As a result, it has been difficult to account for complex mental processes and their place in culture and history. In this startling - indeed, disco in forting - study, Jean Lave moves the analysis of one particular form of cognitive activity, - arithmetic problem-solving - out of the laboratory into the domain of everyday life. In so doing, she shows how (...) mathematics in the 'real world', like all thinking, is shaped by the dynamic encounter between the culturally endowed mind and its total context, a subtle interaction that shapes 1) Both tile human subject and the world within which it acts. The study is focused on mundane daily, activities, such as grocery shopping for 'best buys' in the supermarket, dieting, and so on. Innovative in its method, fascinating in its findings, the research is above all significant in its theoretical contributions. Have offers a cogent critique of conventional cognitive theory, turning for an alternative to recent social theory, and weaving a compelling synthesis from elements of culture theory, theories of practice, and Marxist discourse. The result is a new way of understanding human thought processes, a vision of cognition as the dialectic between persons-acting, and the settings in which their activity is constituted. The book will appeal to anthropologists, for its novel theory of the relation of cognition to culture and context; to cognitive scientists and educational theorists; and to the 'plain folks' who form its subject, and who will recognize themselves in it, a rare accomplishment in the modern social sciences. John Locke is recognized as a great and original thinker, but the attention paid to his philosophy has overshadowed the many other facets of this man's accomplishments in fields such as medicine, botany, economics and literature. The author has gathered together in A Locke Miscellany many unknown essays, articles and descriptive vignettes that offer new views of Locke - both as a multi-talented individual and as an ordinary human being, forced at one time to live in exile.

In Being and Nothingness (1943), Sartre explains love as a strategy for achieving control over "being-for-others," the objectified aspect of the self-imposed by others' defining looks. Two contemporaneous fictions by Sartre, The Room (1939) and Dirty Hands (1948), expand the notions of love and of being-for-others in surprising directions. Dirty Hands shows the creative, productive potential of being-for-others: Hugo's reliance on the other for his self-definition paradoxically generates his decisive embrace of being for-itself. The Room dramatizes the role of the (...) family in constituting a child's subjectivity: Eve's family situation explains her ontological imprisonment in the dimension of being-for-others. The two stories' tolerant vision of the complex social and psychological reasons for adopting being-for-others as one's dominant modality contrasts with Sartre's rigorous critique of reliance on being-for-others as a form of bad faith in Being and Nothingness. The fictions' enlarged perspective on human love and on being-for-others provides a framework for complicating and critiquing the ontological categories presented in Being and Nothingness. (shrink)

Dans les textes de Machiavel, la question de la guerre est souvent l’horizon même de la question de la politique. La politique et la guerre y sont en permanence mêlées, souvent indissociables ; cela dans les relations que les États établissent entre eux, mais aussi à l’intérieur même des États, des provinces et des cités. Il ne s’agit pas ici de couvrir l’ensemble de ce champ des liens entre la politique et la guerre, tâche impossible à mener dans le cadre (...) d’un article, mais seulement de traiter un des aspects de cette question : il s’agira de voir comment la polémique de Machiavel contre les armes mercenaires et auxiliaires et en faveur des « armes propres » est un des axes structurants de l’ensemble de l’œuvre, un des éléments qui donnent sens aux textes. L’erreur consistant à ne pas avoir compris l’importance des « armes propres » et le rôle déterminant de l’infanterie est un des « péchés des princes » de l’époque des guerres d’Italie ; dans l’écriture des textes de Machiavel on ressent un effort constant pour faire comprendre comment on pourrait racheter, « rédimer » (redimere), ces péchés. This challenging and provocative book argues against much contemporary orthodoxy in philosophy and the social sciences by showing why objectivity in the domain of ethics is really no different from the objectivity of scientific knowledge. Many philosophers and social scientists have challenged the idea that we act for objectively authoritative reasons. Jean Hampton takes up the challenge by undermining two central assumptions of this contemporary orthodoxy: that one can understand instrumental reasons without appeal to objective authority, and that the (...) adoption of the scientific world view requires no such appeal. In the course of the book Jean Hampton examines moral realism, the general nature of reason and norms, internalism and externalism, instrumental reasoning, and the expected utility model of practical reasoning. A 62-year-old female with Huntington’s disease presented after a suicide attempt. Her advance directive stated that she did not want intubation or resuscitation, which her family acknowledged and supported. Despite these directives, she was resuscitated in the emergency department and continued to state that she would attempt suicide again. Her suicidality in the face of a chronic and advancing illness, and her prolonged consistency in her desire to take her own life, left careproviders wondering how to provide ethical, respectful care (...) to this patient. Tension between the ethical principles of autonomy and beneficence is central in this case. The patient’s narrative demonstrated that her suicide was an autonomous decision, free from coercion or disordered thinking from mental illness. Beneficence then would seem to necessitate care aligned with the patient’s desire to end her life, which created ethical uneasiness for her family and careproviders. The case highlights several end-of-life ethical considerations that have received much recent attention. With ongoing discussions about the legalization of aid in dying across the country, caregivers are challenged to understand what beneficence means in people with terminal illnesses who want a say in their death. This case also highlights the profound moral distress of families and careproviders that arises in such ethically challenging scenarios. (shrink)

Since the death of Jacques Lacan, Jean Laplanche is now considered to be one of the worlds foremost psychoanalytic thinkers. In spite of the influence of his work over the last thirty years, remarkably little has been available in English. This study uses theories of moral reasoning and moral competence to investigate how university codes of ethics, perceptions of ethical culture, academic pressure from significant others, and ethics pedagogy are related to the moral development of students. Results suggest that ethical codes and student perceptions of such codes affect their perceptions of the ethical nature of the cultures within these institutions. In addition, faculty and student discussion of ethics in business courses is significantly and positively related to moral competence among (...) students. Our results point to the need to further examine the connections among academic institutional structures, ethics pedagogy, and students’ moral development. (shrink)

Long out of print, this is the first English translation in many years. The Imagination is Sartre’s first full philosophical work, presenting some of the basic (...) arguments concerning phenomenology, consciousness and intentionality that were to later appear in his master works and be so influential in the course of twentieth-century philosophy. Sartre begins by criticising philosophical theories of the imagination, particularly those of Descartes, Leibniz and Hume, before establishing his central thesis. Imagination does not involve the perception of ‘mental images’ in any literal sense, Sartre argues, yet reveals some of the fundamental capacities of consciousness. He then reviews psychological theories of the imagination, including a fascinating discussion of the work of Henri Bergson. Sartre argues that the ‘classical conception’ is fundamentally flawed because it begins by conceiving of the imagination as being like perception and then seeks, in vain, to re-establish the difference between the two. Sartre concludes with an important chapter on Husserl’s theory of the imagination which, despite sharing the flaws of earlier approaches, signals a new phenomenological way forward in understanding the imagination. Although written fairly early in his career, in 1939, _Sketch for a Theory of the Emotions_ is considered to be one of Jean-Paul Sartre's most important pieces of writing. It not only anticipates but argues many of the ideas to be found in his famous _Being and Nothingness._ By subjecting the emotion theories of his day to critical analysis, Sartre opened up the world of psychology to new and creative ways of interpreting feelings. Emotions are intentional and strategic ways (...) of coping with difficult situations. We choose to utilize them, _we_ control _them_, and not the other way around, as has been posited elsewhere. Emotions are not fixed; they have no essence and indeed are subject to rapid fluctuations and about-turns. For its witty approach alone, Sartre's _Sketch for a Theory of the Emotions_ can be enjoyed at length. It is a dazzling journey to one of the more intriguing theories of our time. (shrink)
